Genealogy Forms (in Microsoft Word Document "DOC" and Graphic "GIF" Formats)

Please send all completed forms to either your Team Leader and/or Mitch Roll via mail, fax, or e-mail. Your Team Leader will give you specific instructions, and should have their contact information written on each form.

GENERAL OVERVIEW:
Please take every reasonable step to see that the information on all forms is as accurate and complete as practical. This will help maintain the integrity of our database and genealogy book.

You can write any miscellaneous information or additional detail by numbering each note with enclosed parenthesis {"(1)", "(2)", "(3)", etc.}, and then write further information on 1) unused portions of the form; 2) on the reverse side; or 3) on an attached sheet(s).

The trick to reading this printout is to look at the generation numbers. Listed below, Joseph Mischel is the 1st generation and his children, Barbara, Adam, and Elizabeth, are the 2nd generation. All the descendents between A, Adam Mischel, and D, Elizabeth Mischel, are the children, grandchildren, etc of Adam & Marrianna Schwebel Mischel. All the descendents between B, Katherina Mischel, and C, Ferdinand Mischel, are the children of Johann George and Katherina Mischel Roll. Multiple marriages are preceded with an "*".

Family Group Sheet (Form F)
The members of each family (father, mother and children) are recorded on this form. This will be the most frequently used form. Fill out a form on each person who married and list any born or adopted children.

FAMILY GROUP SHEET OVERVIEW:
a) Each time a person DIVORCES and is remarried, you must fill out a separate Family Group Sheet on each marriage. List all children from a previous marriage with a circled capital letter " P " printed in the left margin beside the name, followed by any children born to this couple.
b) Although it is not absolutely critical, please try to list the children CHRONOLOGICALLY with the oldest born listed first and the youngest child as last on the form. This also applies to adopted children.
c) ADOPTED children are acknowledged with a circled capital letter " A " printed in the left margin beside the name.
d) STILLBORNS should also be listed with a given name or state "Infant", "Infant Girl" or "Infant Boy".
e) If the family wishes to list any ILLEGITIMATE children, please place a circled capital letter " I " in the left margin beside the child's name. It would be a good idea to list the father/mother's name/date/place of birth in case the child would someday like to look up the birth parent. All information you wish to remain confidential, please state so on the form.
f) Print a bold " * " by the name of each person you filled out an Ancestor Sheet (Form A).
g) Make sure to use the proper name, date and place formats.


Ancestor Sheet (Form A)
This form is used to list a person's ancestors (spouse, parents, grandparents, great grandparents, etc.). Typically, the spouse marrying into the ROLL family would fill out this form; so we would know who are his/her ancestors (parents, grandparents, etc.). It would not be necessary to fill out the ancestors on the ROLL side because we already researched the ROLLs back a few centuries. We are particularly interested in receiving Ancestor Sheets on spouse's parents who are of German-Russian origin!

Address Sheet (Form E)
Use the Address Sheet to gather your family's addresses, phone numbers and e-mail addresses so we can create a ROLL Family Phone/Address book.

ADDRESS FORM OVERVIEW:
a) DO NOT list anyone who does not have their own household. For example, if the children are still living with the parents, do not list each child with the same address! You only need to list one person per household. Then write down the number of people attending the reunion.
b) With TEMPORARY addresses, print a capital letter " T " in the address box and draw a circle around it.
c) Make sure to use the proper name, date and place formats.
